Medication Tearsheet Tool

Medication Tearsheet Tool

This is a medication sheet doctors can use with their patients when they need to prescribe medication (ACE/ARBs) for high blood pressure.  The sheet was designed  for the doctor to use with patients.  It has questions patients might ask and answers to these questions – similar to the interactive sheet NKDEP developed for eGFR.  The sheet itself tests out at the fifth grade reading level for ease in reading and understanding.

The QIOSC designed this sheet to be a one page, two sided form. QIOs can print it in their office  or send out to be made into a pad with tear sheets.  It can be printed in color or black and white. There is  space on the back sheet under the notes section for each QIO to put their logo and website and phone number, as you wish.
