The latest data update to the New York State Home Health and Hospice Care Profile Web site (homecare.nyhealth.gov) which included updates to quality data, reporting through September 2012, has been published.

Also included in this data update:

  • Updated facilities name, address, counties served, available services, and branches data.
  • Added deemed status for two facilities
  • Updated LCHSA patient hours, CHHA and LTHHCP visits, and hospice patients served by county
  • Updated enforcement actions through March 20, 2012

Quality data and patient survey results refresh on CMS Home Health Compare Website

Home Health quality data on CMS Home Health Compare has been updated as follows:

Reporting data for October 2011 through September 2012:

  1. How often patients got better at walking or moving around.
  2. How often patients got better at getting in and out of bed.
  3. How often patients got better at bathing.
  4. How often the home health team checked patients for pain.
  5. How often the home health team treated their patients’ pain.
  6. How often the home health team treated heart failure (weakening of the heart) patients’ symptoms.
  7. How often patients had less pain when moving around.
  8. How often patients’ breathing improved.
  9. How often the home health team took doctor-ordered action to prevent pressure sores (bed sores).
  10. How often the home health team included treatments to prevent pressure sores (bed sores) in the plan of care.
  11. How often the home health team checked patients for the risk of developing pressure sores (bed sores).
  12. How often patients’ wounds improved or healed after an operation.
  13. How often the home health team began their patients’ care in a timely manner.
  14. How often the home health team taught patients (or their family caregivers) about their drugs.    October 2011     September 2012
  15. How often the home health team checked patients’ risk of falling.
  16. How often the home health team checked patients for depression.
  17. How often the home health team determined whether patients received a flu shot for the current flu season.
  18. How often the home health team determined whether their patients received a pneumococcal vaccine (pneumonia shot).
  19. For patients with diabetes, how often the home health team got doctor’s orders, gave foot care, and taught patients about foot care.
  20. How often patients got better at taking their drugs correctly by mouth.

Reporting data for July 2011 through June 2012:

  1. How often patients receiving home health care needed any urgent, unplanned care in the hospital emergency room – without being admitted to the hospital.
  2. How often home health patients had to be admitted to the hospital.

Quality data updated on CMS Home Health Compare

Home Health quality data has been updated on the CMS Home Health Compare Web site, reporting data for January through December 2011 except for the following measure where data is reported for July 2010 through June 2011:

  • How often patients receiving home health care needed urgent, unplanned care in the emergency room – without being admitted to the hospital
  • Are short of breath less often
  • How often patients’ wounds improved or healed after an operation
  • How often the home health team treated their patients’ pain
  • How often the home health team began their patients’ care in a timely manner
  • How often the home health team checked patient’s risk of falling
  • For patients with diabetes, how often the home health team got doctor’s orders, gave foot care, and taught patients about foot care

Quality data updated on CMS Home Health Compare and Hospital Compare Websites

The latest data update has been published on CMS Home Health Compare, reporting home health quality data for the time period October 1, 2010 through September 31, 2011.

Data has also been refreshed on CMS Hospital Compare as follows:

  • Process of care measures reporting data from April 1, 2010 through March 31, 2011 for all measures except:
    • new heart attack measure – Statin Prescribed at Discharge -  reporting data for January 1, 2011 through March 31, 2011
    • PN 7 – Pneumonia Patients Assessed and Given Influenza Vaccination:  reporting data for October 1, 2010 through March 31, 2011
  • Patient experience data (HCAHPS) updated, reporting data from April 1, 2010 through March 31, 2011
  • Introduction of new hospital acquired infection measure – Central Line Bloodstream Infections – reporting data from January 1, 2011 through March 31, 2011)

Reporting time periods for the following measure sets remains unchanged from previous data refresh on CMS Hospital Compare:

  • Mortality and readmissions measures, reporting data from July 1st, 2007 through June 30th, 2010
  • Medicare reimbursement & number of cases measures, reporting from October 1, 2009 through September 31, 2010
  • Patient safety and hospital acquired conditions measures, reporting from October 1, 2008 through June 30, 2010

SC DHEC releases its 2009 annual report on hospital acquired infections

The South Carolina Department of Health and Environmental Control released its 2009 report online on hospital acquired infection rates in the State. The report, available on the SC DHEC’s Web site, documents all hospital acquired infections and compares them with similar hospitals nationwide.

Wisconsin launches database to assess health care quality, costs

JSOnline writes: A 4-year-old collaborative effort to collect data from Wisconsin’s insurers, hospitals, major employers and others has launched a database that can be used to analyze health systems’ performance.

The WHIO Health Analytics Exchange contains information from millions of insurance claims and is being used by large medical groups, state government, business groups and other members of the Wisconsin Health Information Organization.

Target audience for the database is large medical systems and employee benefit consulting firms initially, with the cost being negotiated on a case-by-case basis. The database will be rolled out to consumers in 2011, to facilitate evaluation of hospitals and individual practice groups on the basis of cost and quality.
